Millhopper Family Dentistry Welcomes you to
our family of fine patients. Thank you for selecting us as
your personal dental care team. We will strive to make your
relationship with us a pleasant one.
A firm foundation is needed on which we can
base recommendations for your dental health; therefore, we
will complete a thorough examination on your first visit.
Dental health is not a one-time affair. A plan
of preventative dentistry is one of the most important services
we have to offer. Keeping your own teeth throughout your lifetime
is possible if you desire it, and we will show you how to
control your own dental destiny.
Preventative examinations on a regularly scheduled
basis will give you the maximum opportunity for a long- term
dental health history. We all need help to recognize diseased
areas and to learn how to keep them free from infection.
A misunderstanding can be an obstacle to forming
a satisfactory relationship. If at any time you have a question
or are unhappy about any treatment fee or service, please
discuss it with us promptly and openly. We would appreciate
this.
After office hours, simply dial the regular
office number. This is intended to provide you with the most
convenient manner in which to contact us. We have a 48 hour
cancellation policy and this answering machine is not intended
for leaving that message unless it is in fact an emergency
situation, and we certainly appreciate that notification.
If you have a dental emergency that cannot wait
until our next working day, follow the instructions on our
answering machine to contact the voice mail number for the
doctor on call, he will be paged and return your call as soon
as possible.
A long-term, mutually satisfying relationship
which gives you the ability to maintain dental health is what
we want for you, your family, friends and our own satisfaction.
Thank you again for selecting us.
Sincerely,
Dr. Kenneth Knopf, Dr. Alecia Skipper McDonald,
Dr. Kirk Juhasz, and Staff
P.S. Please complete the attached information
forms and bring them with you along with your drivers license
to your first visit. If you have dental insurance, please
bring your insurance card and any insurance information you
may have about your policy. A generic insurance claim form
is included here for your convenience should your insurance
company not provide this.
If you have any questions or concerns, feel
free to Contact Us.
