Before your first appointment, please fill out the following forms and bring them with you, along with your driver’s license. Click the links below to download and print the forms.

Payment Options

Payment is expected at the time of service. For easy payments, we offer you the options of paying by cash, credit card, or personal check. You may also pay with a CareCredit credit card which offers a 6-month interest-free payment plan. You can apply for this at our office or online here. We do not participate in any insurance networks at this time.

Directions To Our Office

Our office is located off NW 43rd Street in Gainesville, Florida, south of NW 39th Avenue. On the west side of the road, look for the red brick building and large signage for 3510 – Millhopper Family Dentistry.
